How do I register my child??

• Print out and complete the registration form (to download please click the registration form button)​

• Attach check payable to "SBPAL"

           (see form for the correct amount based on the age of your child)

​If you are new to this PAL league, you must complete ALL the following:​

1. Print out and complete the PAL Basketball Registration Form.

2. Print out and complete the PAL Membership Application Form (please click the button below to download the document)

3. Print out and complete the Code of Conduct Form

4. Attach a copy of the child's birth certificate (for our files)

5. Attach a current "face-shot" photo of the child (such as a school photo)
