All registration forms for basketball can be found on this page once registration has begun for the upcoming basketball season. 

 

How do I register my child??

• Print out and complete the registration form​

• Attach check payable to "SBPAL"

           (see form for the correct amount based on the age of your child)

 

If you are new to this PAL league, you must complete ALL the following:​

1. Print out and complete the PAL Basketball Registration Form.

2. Print out and complete the PAL Membership Application form

3. Print out and complete the Code of Conduct Form

4. Attach a copy of the child's birth certificate (for our files)

5. Attach a current "face-shot" photo of the child (such as a school photo)
